Please use the form below to apply for copies of the SADA Information Pack. Information Packs cost £5 each if you live in Great Britain and Northern Ireland or £10 each if you live overseas.

If you prefer to enrol as a member of SADA, the cost of the Information Pack is included with your membership. You also receive newsletters, list of contacts, research and treatment updates, invitations to meetings, reduced-rate books and other goods. Annual membership costs £12.
